Translation:
Worcestershire: Lewis Whitney gentylman by his attorney against John Williams of the Town of Bergavenny in the county of Monmouth, yeoman, in a plea to pay £5 10s. owed and unjustly detained.

Note: the 'Worcestershire' marginal caption suggests that the the case was begun in that county, but the subject-matter seems to be in the town now called Abergavenny in the Welsh Border country.
